.0 Construction Certificationap ort At the completion of work, a certification report will be prepared summarizing the activities performed at the site. A draft certification report will be completed by the CQA firm and submitted to the County's Resident Engineer no more than 30 days after receipt of the completed as-built survey and will include, at a minimum,the following information: • Brief description of the project including type of facility, name of site, location, altitude, name of County, resident engineer, general contractor, and major subcontractors; Detailed description of the cover and lining systems, including surface area, cross sections, and a summary of materials used; ® Chronological summary of construction activities; • Photographic documentation, including photographs of the site at different phases of construction,photographs of construction details, and photographs of CQA operations; • General record of activities, such as dates of performance of CQA operations, number and names of CQA inspectors; • Geosynthetics manufacturer's data sheets; • Sampling and testing locations; • Copies of CQA data sheets and records completed during construction of the landfill; • CQA field and laboratory test results as well as a summary of these results; • Discussion of special problems encountered and their solutions; ' • A list and discussion of significant changes from design and material specifications; • CQA record survey (as-built) drawings (surveying and base maps required for the ' development of the record drawings are prepared by the project surveyor, and the resident engineer, CQA Engineer, CQA Monitor, and contractors must review and verify that as-builts are correct); and 1 A summary statement sealed and signed by the CQA Engineer documenting that CQA was conducted in accordance with the CQA Plan and, based on visual observations ' and data generated in accordance with the CQA Plan, the landfill and related features shown on the construction drawings were constructed in accordance with project construction drawings and specifications except as properly authorized and ' documented in the CQA Final Report.
