Include logs of wells. <br />Please submit the information from Sections 4.1 and 4.2 <br />which address this requirement. <br />(m) Submit a copy of the letter from PG&E regarding landfill gas <br />migration. The letter should be included as an appendix of <br />the RDSI. <br />(o) 3. List the names, addresses and telephone numbers of <br />persons to be notified in case of emergency and of all <br />landowners. <br />(p) 1. List and include a copy of all permits, requirements, <br />etc. of other agencies regarding this site. This <br />includes but not limited to the approval letter from <br />the Stockton Fire Department and the WDR. Please note <br />that it is required that all permits and approvals be <br />obtained prior to Board staff recommending concurrence <br />in a SWFP. <br />Permit Review Report (PRR) <br />The October 3, 1989 PRR finds that there have been <br />significant change at the subject site. I agree with the <br />determination due to the increase in permitted daily <br />capacity from 100 tons per day to 470 tons per day. <br />Periodic Site Review (PSR) <br />The PSR made several references to the RDSI. It is the policy of <br />this Board to require PSR's to be "stand-alone" documents. <br />Furthermore, 14 CCR 17607 requires that the PSR include the <br />estimated remaining site life, conclusion, and recommendations, <br />none of which was provided. <br />When resubmitting the PSR, operators may either 1) resubmit a new <br />copy of the revised PSR or 2) submit revised pages for insertion <br />into the current document. A revised title page indicating the <br />original PSR date and all revision numbers and revision dates is <br />required. A new table of contents page, if necessary, is also <br />required. <br />
