April 4, 2003 Version <br /> 1 (4)A statement by the owner and operator certifying under penalty of perjury that the <br /> 2 information which they have provided is true and accurate to the best of their knowledge and <br /> 3 belief. <br /> 4 (b) The notification shall be mailed to the EA "return receipt requested." <br /> 5 <br /> 6 NOTE: Authority cited: Sections 40502 and 43020 of the Public Resources Code. <br /> 7 Reference: Sections 43020 of the Public Resources Code. <br /> 8 <br /> 9 Article 3.2. Reports of Facility Information <br /> 10 <br /> 11 Section 18227. Report of Composting Site Information <br /> 12 <br /> 13 Each operator of a compostable material handling facility that is required to obtain a <br /> 14 Compostable Materials Handling Facility Permit, as specified in Title 27, California Code of <br /> 15 Regulations,Division 2, Subdivision 1, Chapter 4, Subchapter 1 and Subchapter 3,Articles <br /> 16 1,2,3, and 3.1 (commencing with section 21450), shall, at the time of application, file a Report of <br /> 17 Composting Site Information with the EA as required by Section 17863 of this Title. A Report <br /> 18 of Composting Site Information shall contain the following: <br /> 19 (a) A description of the processes to be used, including estimated quantities of feedstocks, <br /> 20 additives, and amendments. <br /> 21 (b)A descriptive statement of the operations conducted at the facility. <br /> 22 (c)A schematic drawing of the facility showing layout and general dimensions of all processes <br /> 23 utilized in the production of compost including,but not limited to, unloading, storage, <br /> 24 processing,parking, and loading areas. <br /> 25 (d)A description of the proposed methods used to control leachate, litter, odors, dust,rodents, <br /> 26 and insects. <br /> 27 (e) A description of the proposed emergency provisions for equipment breakdown or power <br /> 28 failure. <br /> 29 (f) A description of the storage capacity and anticipated maximum and average length of time <br /> 30 compostable materials will be stored at the facility. <br /> 31 (g) A description of compostable materials handling equipment used at the facility including <br /> 32 type, capacity, and number of units. <br /> 33 (h)Anticipated annual operation capacity for the facility in cubic-yards. <br /> 34 (i) A description of provisions to handle unusual peak loadings. <br /> 35 0)A description of the proposed method for storage and final disposal of nonrecoverable or <br /> 36 nonmarketable residues. <br /> 37 (k) A description of the water supplies for process water required. <br /> 38 (1) Identification of person(s) responsible for oversight of facility operations. <br /> 39 (m)A description of the proposed site restoration activities, in accordance with Section 17870. <br /> 40 (n) An Odor Impact Minimization Plan pursuant to Section 17863.4. <br /> 41 <br /> 42 Authority cited: Sections 40502, 43020, 43021 and 43209.1 of the Public Resources Code <br /> 43 Reference: Sections 43020, 43021 and 43209.1 of the Public Resources Code. <br /> 44 <br /> 20 <br />
