This <br />system conveys chemical identity by chemical or common names, code numbers or other <br />descriptive terms, which clearly identify the material for hazard information purposes. The acute <br />health, flammability, and reactivity hazards are communicated by numerical ratings insetted on <br />the label itself <br />Hazard Index (141) <br />The description of the numerical Hazard Index (HI) is shown as follows - <br />4 Severe Hazard <br />3 Serious Hazard <br />2 Moderate Hazard <br />I Slight Hazard <br />0 Minimal Hazard <br />These designations will be inserted (written) with indelible ink by each hazard on the label, i.e., <br />Health, Flammability, Reactivity, That information is obtained from the MSDS or other sources <br />by the Regional Safety & Health Office or Facility Safety Representative. <br />Definitions of Hazard Categories: <br />Health Hazard (Blue Section) <br />4 Danger: May be fatal on short exposure. Specialized protective equipment <br />required. <br />3 Warning: Corrosive or Toxic — avoid skin contact or inhalation. <br />2 Warning: May be harmful if inhaled or absorbed, <br />I Caution: May cause irritation <br />0 No unusual hazard <br />Fire Hazard (Red Section) <br />4 Danger: Flammable gas or extremely flammable liquid. <br />3 Warning: Flammable liquid, flash point below 100 degrees F. <br />2 Caution: Combustible liquid, flash point of 100 degrees F to 200 degrees F. <br />I Combustible if heated. <br />0 Not combustible. <br />Page 8 of 9 <br />0 2009 Republic Services Mazard Communication program <br />
