The following provisions must be met : <br /> Incoming records of weights or volumes to within 10% <br /> accuracy must be instituted by the operator by March 1, <br /> 1978 . <br /> ..- A location Of all accurate records pertaining to the <br /> station operations must be provided to the San Joaquin <br /> Local Health District by March 1 , 1978. <br /> Provide identification signs at each point of access to <br /> the station indicating the name of the station operator <br /> by April 1 , 1978. <br /> d Internal roads and the rear equipment storage area must <br /> be made smooth and designed to minimize the generation of <br /> dust and the tracking of materials onto adjacent paved <br /> public roads by September 1, 1978, <br /> Adequate handwash facilities must be provided for station <br /> employees by April 1, 1978, <br /> J/ Approved safety helmets and respirators must be provided <br /> and used by all station equipment operators by March 1, 1978 . <br /> g;. Litter control equipment must be provided around the portable <br /> se.affoldirz area where loaded vans are covered by April 1, <br /> 1978 . <br /> h. A safe area must be set aside for burning wastes received <br /> and a means of extinguishing these burning wastes in this <br /> area by March 1, 1978. <br /> , p, Debris accumulated in the rear storage area must be removed <br /> by March 1 , 1978. <br /> 2 . This permit is subject to review by the San Joaquin Local Health <br /> District, and may be suspended, revoked, or modified at any time <br /> for sufficient cause. <br /> E. Self-Monitoring Program: <br /> 1. The following items shall be monitored by the operator of .the <br /> facility or his agent and records shall be kept and made available <br /> to the San Joaquin Local Health District upon request : <br /> a. Number of vehicles utilizing the site per day and per week. <br /> b . Quantity and types of wastes received at the site per day <br /> and per week. <br /> C. Quantity and types of wastes salvaged per week. <br />
