The following provisions must be met : Incoming records of weights or volumes to within 10% accuracy must be instituted by the operator by March 1, 1978 . ..- A location Of all accurate records pertaining to the station operations must be provided to the San Joaquin Local Health District by March 1 , 1978. Provide identification signs at each point of access to the station indicating the name of the station operator by April 1 , 1978. d Internal roads and the rear equipment storage area must be made smooth and designed to minimize the generation of dust and the tracking of materials onto adjacent paved public roads by September 1, 1978, Adequate handwash facilities must be provided for station employees by April 1, 1978, J/ Approved safety helmets and respirators must be provided and used by all station equipment operators by March 1, 1978 . g;. Litter control equipment must be provided around the portable se.affoldirz area where loaded vans are covered by April 1, 1978 . h. A safe area must be set aside for burning wastes received and a means of extinguishing these burning wastes in this area by March 1, 1978. , p, Debris accumulated in the rear storage area must be removed by March 1 , 1978. 2 . This permit is subject to review by the San Joaquin Local Health District, and may be suspended, revoked, or modified at any time for sufficient cause. E. Self-Monitoring Program: 1. The following items shall be monitored by the operator of .the facility or his agent and records shall be kept and made available to the San Joaquin Local Health District upon request : a. Number of vehicles utilizing the site per day and per week. b . Quantity and types of wastes received at the site per day and per week. C. Quantity and types of wastes salvaged per week.
