Results of all self-monitoring programs,as described in the Report of Facility Information,will be reported as follows: <br /> Program Reporting Facility Agency Reported To <br /> 1. Log of Special Occurrences(i.e.tires,explosions, KEEP ON SITE AND AVAILABLE LOCAL ENFORCEMENT AGENCY <br /> accidents,inadvertent acceptance of hazardous wastes, UPON REQUEST OF LEA (LFA) <br /> etc...)must be kept on site and made available upon <br /> request. <br /> 2. An employee training log with course descriptions,dates KEEP ON SPIE AND AVAILABLE LEA <br /> of training,etc..shall be maintained and kept current. UPON REQUEST OF LEA <br /> 3. A report of quantities and types of wastes received and KEEP ON SITE AND AVAILABLE LEA <br /> product removed from the facility shall be provided to UPON REQUEST OF LEA <br /> the LFA upon request (showing monthly breakdowns). <br /> The records shall indicate the quantities in ton/cubic <br /> yards and include weight and record data. Records shall <br /> be kept in one location and accessible for five(5)years. <br /> 4. One sample per 5000 cy of finished compost must be PER 5000 CY OF FINISHED COMPOST LEA <br /> taken and analyzed at an approved Department of Health <br /> Services Laboratory. Samples shall be analyzed for <br /> constituents required pursuant to 14 CCR,Section <br /> 17868.1. <br /> KEEP ON SITE AND AVAILABLE LEA <br /> �.
