Log of Special Occurrences(i.e.tires,explosions,accidents,inadvertent e. ANNUALLY TO LEA <br /> acceptance of hazardous waste,etc. .)must be!rept on site and submitted to <br /> LEA annually. <br /> b. An employee training log with course descriptions.dates of training,etc...shall b. KEEP ON SITE&AVAILABLE <br /> be maintained and kept current, UPON REQUEST OF LEA <br /> C. A record of weather conditions shall be maintained with operation records c. KEEP ON SITE&AVAILABLE <br /> pursuant to 14 CCR Section 17859. UPON REQUEST OF LEA <br /> d. A report of quantities and t%pes of waste received and product removed from d. QUARTERLY TO LEA <br /> the facility shall be provided to the LEA on a quarterly basics(showing <br /> monthly breakdowns). The records shall indicate the quantities in tons!cubic <br /> yards and include weight and recon data. Records shah be submitted to the <br /> LEA on the 15u'day of the subsequent month. <br /> e.
