(Blaes), on behalf of Amerco Real Estate Company <br /> /U-Haul International, is submitting Well Permit Applications and an associated work plan to the <br /> San Joaquin County Public Health Services, Environmental Health Division for review apd <br /> approval. The Well Permit Applications are for additional site characterization scheduled for the <br /> above-mentioned site and include the following items: <br /> 1) Two Well Permit Applications - one for well installation and one for soil sampling; <br /> 2) Work Plan with site plan showing the proposed location of monitor wells and soil borings; <br /> 3) Site Specific Health & Safety Plan; and <br /> 4) A check for application fees totaling$178.00. <br /> As part of the permit application, the "Licensed Contractors Declaration" form for the two <br /> scheduled drilling contractors will be faxed and mailed to your office before work begins.
