' Ensure the Health&Safety Plan(HASP)Approval Form in Attachment I is completed and appended to the HASP, PP • Obtain for each Field Team Member documentation of that member's formal training required by the Program The Project Manager shall include this documentation in the project file, ' Ensure that Field Team Member receives site-specific training as required by the HASP, • Obtain for each Field Team Member the date that Member's medical surveillance is next due The tProject manager shall note this date in the project file for each Field Team Member If the HASP requires additional medical momtonng,the Project Manager shall ensure each Field Team Member receives the additional monitoring ' Ensures that the health&safety equipment required by the HASP is available,is in proper working condition and if required,is properly calibrated prior to use ' Required equipment shall include,as appropriate -monitoring instruments, ' -personal protective equipment, -decontamination equipment and supplies, -emergency firefighting and first aid equipment, ' -sanitation facilities and equipment, -illumination equipment/portable hghting,if necessary, -fled communication equipment, -site control barricades,tape, stakes,fences(if appropriate),and -spill control equipment Appoint at Ieast one qualified individual to serve as Site Health and Safety Officer(SHSO), • Appoint qualified individuals to serve as Field Team Leader(s) The Project Manager may designate one Field Team Member to serve as both Field Team Leader and SHSO if anticipated site contaminants can be controlled using Level D or Level C protection, and if the size and scope of field activities permit one Field Team Member to perform both functions, Ensure that at least one Field Team Member with current First Aid/CPR training is on-site when field activities are being performed, 2 2 SITE HEALTH AND SAFETY OFFICER(SHSO) The SHSO shall carry out the responsibilities designated to that position as specified in the HASP Specifically those responsibilities include Ensure health and safety instruments used to determine the Level of Protection arer p openly calibrated and used in accordance with applicable calibration procedures, .
