The following is an itemized list of underground storage tank violations that have not been addressed for EI Dorado Gas & Mart as of May 01 , 2020 . Open violations from May 31, 2019 inspection Violation #103 - Failure to submit and maintain complete and current financial responsibility documents . Financial responsibility documents have not been submitted to the EHD . -The signatures and asset/liability amounts on the Certification of Financial Responsibility and Letter from the Chief Financial Officer were not renewed . -The dates next to the signatures on the previous forms were whited out and rewritten . -The most recent financial responsibility documents are dated 3/5/ 18 . Current financial responsibility documents are required to be submitted annually. Immediately log into the California Environmental Reporting System (CERS ) at http : //cers. calepa . ca . gov/, and upload the required documents . ❑ This violation was corrected ❑ This violation will be corrected by ( date) : ❑ Supporting documents included Describe actions taken or will be taken to correct violation : Violation #105 - Failed to have an approved UST monitoring plan . The monitoring plan is not current and/or not approved by the EHD . -The plans incorrectly state that UDC sensor leak alarms and disconnection/failure trigger pump shutdown . The monitoring plan must be uploaded to the California Environmental Reporting System (CERS ) . Immediately log into CERS , make the necessary changes , and submit for review by the EHD . ❑ This violation was corrected ❑ This violation will be corrected by (date) : ❑ Supporting documents included Describe actions taken or will be taken to correct violation : Violation #201 - Failure to maintain monitoring , maintenance, and follow up action records . Maintenance and monitoring records for the last three years were not found on site . -The 2016 monitoring system certification test results were not found on site . -The 2017 monitoring system certification test results were not found on site . -The 2018 monitoring system certification test results were not found on site . -The 2017 secondary containment test results were not found on site . These records shall be maintained on site for at least three years . Monitoring records include : ( 1 ) date and time of all monitoring or sampling ; (2 ) monitoring equipment calibration and maintenance records ; (3) results of any visual observations ; (4) results of sample analysis performed a lab or in the field ; (5) logs of all readings of gauges or other monitoring equipment , ground water elevations , or other test results ; (6) results of inventory readings and reconciliations .
