The four <br /> monitoring wells that have been installed at this site for the <br /> groundwater investigation are therefore no longer needed and <br /> should be properly destroyed under permit from PHS/EHD. The <br /> proper destruction of these wells is required so that they will <br /> not pose a future threat to groundwater quality. <br /> Please submit the enclosed permit application (to be completed by <br /> a licensed contractor) and the $60. 00 destruction fee to PHS/EHD <br /> within 30 days from the date of this letter.
