Hazardous Waste Contingency Plan <br /> • Time and type of incident (for example , release , fire) ; <br /> • Name and quantity of material ( s) involved , to the extent known ; <br /> • The extent of injuries , if any ; and , <br /> • The possible hazards to human health , or the environment , outside the Site . <br /> Per 22 CCR 66265 . 56 (d ) ( 2 ) , the emergency coordinator will immediately notify CalEPA Office of <br /> Emergency Services in every situation and will report : <br /> • Name and telephone number of the reporter ; <br /> • Name and address of the facility ; <br /> • Time and type of incident ( e . g . , release , fire) ; <br /> • Name and quantity of material ( s) involved , to the extent known ; <br /> • The extent of injuries , if any ; and , <br /> • The possible hazards to human health , or the environment , outside the facility . <br /> M Evacuation <br /> If directed by the Emergency Coordinator to begin evacuation , Site personnel will evacuate using the <br /> designated routes , <br /> b . 5 i erinination and Follow- Up Acriunb <br /> Once the incident is fully under control , the Emergency Coordinator will direct appropriate cleanup <br /> and disposal measures . The Emergency Coordinator must provide for treating , storing , or disposing <br /> of recovered waste , contaminated soil or surface water, or any other material that results from a <br /> release , fire , or explosion at the Site . <br /> The Emergency Coordinator must also ensure that emergency response equipment listed in the Plan <br /> is cleaned and ready for its intended use before Site operations resume . Emergency response <br /> equipment will be inventoried , and any necessary replacement equipment will be obtained . <br /> 9 . 6 Written Report <br /> Within 15 days after an incident that requires implementation of the contingency plan , a written report <br /> on the incident will be submitted to the EPA Regional Administrator and the CalEPA . <br /> The report will include : <br /> • Name , address , and telephone number of the Site and owner/operator ; <br /> • Date , time , and type of incident ( e . g . , fire , explosion ) ; <br /> • Name and quantity of material (s) involved ; <br /> • The extent of injuries , if any ; <br /> arcadis . com 13 <br />
