San Joaquin County <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone:(209)468-3420 Fax:(209)468-3433 Web:www.slgov.orq/ehd <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> RITE AID#6005 1245 W YOSEMITE AVE, MANTECA December 04, 2018 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 605 CCR 66262.34(f) Failed to completely label containers or portable tanks of hazardous waste. <br /> The following observation was made in the pharmacy: <br /> -The pharmacy manager,Junabel Pedro,stated that loose pills are processed monthly and are then placed into the <br /> pharmacy's hazardous waste bin.When asked where the loose pills are held before they are processed,Junabel <br /> grabbed a container from the counter and stated the pills are held in the container.The container was open, <br /> unlabeled, and contained bagged loose pills, some of which were labeled with NDC numbers(IMG 2909-2910). <br /> All hazardous waste containers shall be marked with the following information: <br /> -the words"Hazardous Waste" <br /> -name and address of generator <br /> -hazardous properties <br /> -physical state <br /> -composition(contents) <br /> -accumulation start date <br /> Immediately label these containers and ensure that all hazardous waste containers are marked with all the required <br /> information. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> An inspection checklist was provided to the facility operator on the day of inspection.
