The generator manifest copy shall be mailed to: <br /> DTSC Generator Manifests <br /> P.O. Box 400 <br /> Sacramento, CA 95812-0400 <br /> Immediately mail a copy of the manifests listed and ensure that copies of all generator manifests are sent to DTSC <br /> within 30 days of shipping hazardous waste. <br /> This is a Class II violation. <br /> 203 HSC 25189.5(c)Transported or caused transportation of hazardous waste to an unauthorized point. <br /> The following observation was made in the pharmacy: <br /> -Documentation titled"Closed Transfer Detail Listing"listed Warfarin Sodium,which was sent to Med-Turn Inc. on <br /> 9/7/18 and 11/21/18,as"Damaged/Outdated"(IMG 2912-2913). Med-Turn Inc. does not have a permit from the <br /> Department of Toxic Substance control (DTSC)to receive hazardous waste. <br /> No person shall transport or cause transportation of hazardous waste to a facility which does not have a permit from <br /> DTSC. <br /> Immediately cease the transportation of hazardous waste to unauthorized locations. <br /> This is a Class II violation. <br /> 403 CCR 66262.34(d)(2) Failed to keep hazardous waste containers closed except when adding or removing hazardous <br /> waste. <br /> The following observation was made in the pharmacy: <br /> -The pharmacy manager,Junabel Pedro, stated that loose pills are processed monthly and are then placed into the <br /> pharmacy's hazardous waste bin.When asked where the loose pills are held before they are processed, Junabel <br /> grabbed a container from the counter and stated the pills are held in the container.
