DOCUMENTATION <br /> Sample Container) bras <br /> Each sample container shall be labeled immediately after the sample is collected and sealed. <br /> The label shall include. <br /> • Company Name(HETI) <br /> • Source(i e,well number or code) <br /> • Sampler's identification <br /> • Project number <br /> • Date and time of collection <br /> • Type of preservation (if any)used <br /> In the field, the HETI sampling crew will record the following information on the <br /> Purge/Sample Sheet (Plate A-2) for each well sampled: <br /> • Project number <br /> • Client <br /> • Location <br /> • Source(i.e,well number or code) <br /> • Time and date of development,purging and sampling <br /> • Well accessibility and integrity <br /> • Pertinent well data (e g, total depth, product thickness, static water <br /> level) <br /> • Physical parameters when appropriate (e.g, specific conductance, pH, <br /> temperature)-may be more than one reading <br /> • Gallons and well casing volumes purged, - <br /> Chain-of-Custody <br /> A chain-0f-custody record shall be completed and will accompany every shipment of samples to <br /> the analytical laboratory in order to establish documentation tracing sample possession from <br /> the time of collection until delivery to the laboratory The record will contain the following <br /> information <br /> • Sample or station number or code(ID) <br /> • Signature of the collector, sampler,or recorder <br /> • Date and time of collection <br /> • Place of collection(project address and name of business) <br /> • Sample type (soil or water) <br /> • Type of analysis requested <br /> • Signatures of persons involved in chain of possession(in <br /> chronological order) <br /> • Dates and times of individual possession(inclusive) <br /> • Laboratory comments regarding the sample receptacle conditions <br /> Samples will always be accompanied by a Chain-of-Custody record When transferring the <br /> samples, the individuals relinquishing and receiving the samples will sign, date and note the <br /> time on the Chain-of-Custody record <br /> 17 <br />
