OBSERVATION: The following maintenance and monitoring record was not found on site: -According to the replacement notification sent to EHD on 3/26/2020,the L15 sensor was replaced on 3/26/2020. Work orders with the description of the parts installed, name and UST ICC number of the service technician and testing documentation verifying the installation and functionality of the repair was not found at the time of the inspection. REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records include: (1)date and time of all monitoring or sampling; (2)monitoring equipment calibration and maintenance records; (3)results of any visual observations; (4)results of sample analysis performed a lab or in the field; (5)logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6)results of inventory readings and reconciliations. CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the last three years on site and submit copies to the EHD. This is a repeat violation, Class II. 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. 1)OBSERVATION:The designated operator failed to check and note the correct test dates for the overfill prevention testing on the designated operator inspection reports. Reports stated overfill prevention testing was conducted on 6/19/2019; according to results submitted to EHD, testing was completed on 7/19/2019 and 11/13/2019. REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. 2)OBSERVATION:The designated operator failed to check and note all required items on the visual inspection reports.The incorrect and missing information included the following from the inspection reports: -The"NA"in section X was not explained in section III of the reports. -The reports were missing CERS ID. -Sections VII and VIII were left blank. REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II.
