In addition— pursuant to the San Joaquin Local Health District fee -schedule Resolution No. 85-58, a $35.00 consultation fee shall be submitted along with the completed general Environmental Health Application Form for review of the above referenced Soil Remediation Workplan, and a reinspection fee of $35.00 per hour for the sampling verification inspection. Each additional hour over the initial one hour of time spent in review or inspection activity will be billed at the rate of $35.00 per hour. The additional amount due will be billed to the applicant identified on the general Environmental Health Application Form at the completion of requested activity, or monthly, if longer than 30 days is required for completion.
