PHS/EHD <br /> has the following comments for your consideration. <br /> The addendum proposed to install two monitoring wells to supplement MW3 and MW4, <br /> each with twenty feet of screened casing, to be placed ten feet below the groundwater <br /> table and ten feet above the groundwater table. <br /> The addendum included altered locations for the proposed monitoring wells. The <br /> statement was also included in the addendum to indicate that PHS/EHD would have an <br /> opportunity to review the selection of soil samples prior to laboratory submittal. <br /> PHSIEHD grants approval of the "Addendum to Work Plan - dated 23 August 1996" and <br /> the "Monitoring Well Installation Work Plan". Please proceed with submittal of a drilling <br /> permit application and $89 fee. Field work may be scheduled upon permit issuance. <br /> Please contact me at least two working days prior to implementing the approved scope of <br /> work.
