Isabel Foranda indicated that she was only aware of <br /> where the manifests are kept in the facility-in a manila folder in the facility office. <br /> A generator copy of manifest number 010788935FLE(12/21/17)(IMG 6059)was not sent to DTSC within 30 days of <br /> shipment of hazardous waste. <br /> Generator copies of the following manifests numbers are not on file with DTSC: <br /> -012039705FLE (10/3/18)(IMG 6060) <br /> -012039704FLE (10/3/18)(IMG 6061) <br /> -012951795FLE (1/31/19)(IMG 6058) <br /> -012951794FLE (1/31/19)(IMG 6057) <br /> The generator manifest copy shall be submitted to DTSC for every shipment on a manifest when California is either the <br /> generator state or the destination or consignment state. The generator manifest copy shall be mailed to: <br /> DTSC Generator Manifests <br /> P.O. Box 400 <br /> Sacramento, CA 95812-0400 <br /> Immediately mail a copy of manifest 010788935FLE(12/21/17), as well as the other manifests listed if needed, and <br /> ensure that copies of all generator manifests are sent to DTSC within 30 days of shipping hazardous waste. <br /> This is a Class II violation. <br /> 403 CCR 66262.34(d)(2) Failed to keep hazardous waste containers closed except when adding or removing hazardous <br /> waste. <br /> Approximately nine empty packs of GS Neutral Cleaner Concentrate were found un-bagged on a shelf inside the <br /> cleaning chemical storage cabinet adjacent the HW storage area.A HW bag labeled STOU Store Use, containing <br /> several empty packs of GS Neutral Cleaner Concentrate,was previously found in a STOU HW tote with accumulation <br /> start date 4/12/19. Isabel placed the nine empty packs of GS Neutral Cleaner Concentrate into the HW bag <br /> containing the cleaner packs, and into the STOU HW.
