SAN" J D�l+ �� I 1411" Environmental Health Department <br /> COUNTY�l <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> DOLLAR TREE#05193 1 10408 TRINITY PKWY STOCKTON May 08 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 405 CCR 66262.34(d)(2) Failed to manage incompatible wastes in containers properly. <br /> -The following items were found in a STOU HW tote with accumulation start date 4/12/19(IMG 6047-53): <br /> o Two bags labeled STOU HW, one containing two 1.4 fl. oz.tubes of Coloreazy Permanent Cream Hair Color, and <br /> the other containing two tubes of Coloreazy Permanent Cream Hair Color and two 1.4 fl. oz. containers of Coloreazy <br /> Developer.According to Isabel, the cream hair color and the developer come together in a box, and when the barcode <br /> on the box is scanned, the scanner states the items in the box are to be placed in an STOU HW tote.According to <br /> the ingredient list,the cream hair color contains ammonium hydroxide,while the developer contains hydrogen <br /> peroxide.According to Dollar Tree's California Training Guide on Hazardous Waste Handling and Processing <br /> Procedures(version 9.12.14), items containing ammonium hydroxide are classified as CB and items containing <br /> hydrogen peroxide are classified as OX. <br /> Also, according to a poster titled"Rules for Accumulating Hazardous Waste"posted behind the HW storage area, <br /> "Rule#1" indicates only one type of waste/waste item should be placed into any one bag. <br /> Isabel transferred the cream hair color tubes into one bag and placed them into a CB HW tote, and placed the <br /> developers into another bag and into an OX HW tote. <br /> Incompatible wastes shall not be placed in the same container, or in an unwashed container that previously held an <br /> incompatible waste or material.
