Isabel placed the nine empty packs of GS Neutral Cleaner Concentrate into the HW bag <br /> containing the cleaner packs, and into the STOU HW. (IMG 6004, 6013-7). <br /> All hazardous waste containers shall be marked with the following information: <br /> -the words"Hazardous Waste" <br /> -name and address of generator <br /> -hazardous properties <br /> -physical state <br /> -composition (contents) <br /> -accumulation start date <br /> Immediately label these containers and ensure that all hazardous waste containers are marked with all the required <br /> information. <br /> This was corrected on site. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Note: <br /> An inspection checklist was provided to the facility operator on the day of inspection. <br /> The EHD has amended and written the complete report which replaces the initial checklist on 5/20/2019. <br /> Violation 109 was added because a generator copy of manifest number 010788935FLE (12/21/17)was not sent <br /> to DTSC within 30 days of shipment of hazardous waste. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 6/19/2019. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
