Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> #23 Rodents, Insects or Animals Inside Facility MAJOR <br /> OBSERVATIONS:While on site, it was observed of multiple rest located inside of the facility. The ware-washing sink <br /> contained multiple dead roaches on the sink bed, roaches were found inside a prep cooler on the chef line and in multiple <br /> crevices throughout the facility. In addition, rodent droppings were also located on site, in multiple locations. Manager stated <br /> that facility contains a pest control service,which visited the facility in April according to the invoice. Continue implementing <br /> pest control, remove and sanitize all surfaces containing roaches or droppings. Correct immediately. <br /> CALCODE DESCRIPTION:Each food facility shall be kept free of vermin:rodents(rats,mice), cockroaches,flies.(114259.1, 114259.4, <br /> 114259.5) <br /> #1 Demonstration of Knowledge <br /> OBSERVATIONS:Observed expired food handler cards while on site. Manager confirmed associates are still currently <br /> employed on site. Obtain renewed copies and provide to Victor Acevedo(vmacevedo@sjgov.org)within 30 days. <br /> CALCODE DESCRIPTION:All food employees shall have adequate knowledge of and be trained in food safety as it relates to their <br /> assigned duties.
