Kennedy/Jenks Consultants <br /> 1.0 INTRODUCTION <br /> This Site Safety and Health Plan, developed in accordance with OSHA standards for <br /> hazardous waste operations (29 CFR 1910 120), establishes general health and safety <br /> protocols for Kennedy/Jenks Consultants (K/J) personnel at 102 S Wilson Way, Stockton, <br /> California. Addenda containing activity- specific health and safety protocols will be <br /> prepared and attached to this Site Safety and Health Plan prior to the initiation of each <br /> substantially different field activity The Site Safety and Health Plan and activity specific <br /> Addenda, as a minimum, will contain the following information <br /> (A) Names of key personnel and alternates responsible for site safety and <br /> health and appointment of a Site Safety Officer <br /> (B) A safety and health risk evaluation for each site task and operation <br /> (C) Personal protective equipment to be used by employees for each site task <br /> and operations being conducted <br /> (D) Medical surveillance requirements <br /> (E) Frequency and types of air monitoring, personal monitoring and <br /> environmental sampling techniques and instrumentation to be used <br /> Methods of maintenance and calibration of monitoring and sampling <br /> equipment to be used <br /> (F) Site control measures <br /> (G) Decontamination procedures <br /> r <br /> (H) Site's standard operating procedures <br /> (1) An Emergency Response Plan which addresses effective site response to <br /> emergencies As a minimum, the elements of the Emergency Response <br /> Plan will include the following <br /> (1) Pre-emergency planning <br /> (2) Personnel roles, lines of authority training and communication <br /> (3) Emergency recognition and prevention <br /> (4) Safe distances and places of refuge <br /> (5) Site security and control <br /> (6) Evacuation routes and procedures <br /> (7) Decontamination <br /> 51 mptm&splan 1 91055700 <br />
