San Joaquin County <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone:(209)468-3420 Fax:(209)468-3433 Web:www.slaov.org/ehd <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> 99 Cents Only Stores#177 1120 E Hammer Ln, Stockton February 07, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 106 CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br /> Rabia Mahmood,the store manager, provided a record of 2019 employee hazardous waste management <br /> (computer-based)training for the following employees:Victoria Antonio,Jose Castro,Albany Ham, Muhammad <br /> Khan, Blia Lee, Rabia Mahmood, Soriya Muth, Katrina Sanchez, Mai See Yang, and Igra Zahid (see photo IMG <br /> 4233). Rabia also stated that not all of the remaining store employees have completed the new 2019 employee <br /> hazardous waste training. <br /> Based on the types of violations observed,the facility employees are not trained on proper hazardous waste(HW) <br /> handling or the training provided has been inadequate. <br /> The following observations were made at the time of inspection: <br /> -All of the HW totes containing waste and labeled as HW in the HW storage area(IMG 4130)did not have the <br /> name of the generator on the label, nor were the contents of each tote marked on the labels, although the labels <br /> state"Contents: (Check appropriate boxes below)"and then have a checklist of items.The HW totes that did not <br /> have the name of the generator, nor the contents marked on the labels were the following: <br /> -A HW Oxidizers tote with accumulation start date 1/8/19(IMG 4132). <br /> -A HW Oxidizers tote with accumulation start date 1/8/18(IMG 4135). <br /> -A HW Corrosive Bases tote with accumulation start date 10/10/18 (IMG 4165). <br /> -A HW Aerosols, Flammables and Combustibles(Flammables)tote with accumulation start date 10/10/18(IMG <br /> 4193). <br /> -A HW Corrosive Acidic tote lacking an accumulation start date (IMG 4197). Rabia confirmed that the items in this <br /> tote have not been stored on site as hazardous waste for over 180 days, per manifest 012120116FLE(dated <br /> 10/4/18)(IMG 4234),which included corrosive acidic liquids for disposal. <br /> -A HW Flammables tote with accumulation start date 11/14/18, lacked the generator's address, and contained <br /> several items including a box of Breezy dryer sheets and a box of Suavitel dryer sheets (IMG 4212-4214). <br /> -The following item was found in the HW Oxidizers tote with accumulation start date 1/8/19(IMG 4132): <br /> -A container of LA's Totally Awesome drain opener with absorbent.The first ingredient in the drain opener is <br /> sodium hydroxide,which according to the 99 Cents Only Stores Hazardous Waste poster(The HW Poster)posted <br /> above the HW storage area, is an ingredient that is classified as a Corrosive Base (IMG 4215). <br /> -The following items were found in the HW Oxidizers tote with accumulation start date 1/8/18(IMG 4135): <br /> -An aerosol can of Professional Sebastian Color Ignite Single Tone labeled as flammable was bagged with <br /> Colgate Triple Action toothpaste, a container of hydrogen peroxide(an oxidizer according to The HW Poster), and <br /> two liquid air fresheners(IMG 4136-4138). However,The 99 Cents Only Stores Rules For Accumulating Hazardous <br /> Waste poster(The Accumulation Poster), posted above the HW storage area, states"One Waste-One Bag," <br /> indicating only one type/item of waste should be placed in a single bag (IMG 4216). Rabia Mahmood,the store <br /> manager, placed the aerosol can in a HW Flammables tote at the time of inspection.When asked why the <br /> toothpaste and the air fresheners were placed in the Oxidizers tote, Rabia did not know.According to the 99 Cents <br /> Only Stores Hazardous Waste Management CA 2019 employee training material, toothpaste is classified as a toxic <br /> waste, and to be placed in the HW Corrosive Bases tote. <br /> -A leaking container of Ensueno Original Spring Fresh Fabric Softener bagged without absorbent(IMG 4145), <br /> although according to The Accumulation Poster"Rule#2"states"No Free Liquids"and indicates absorbent should <br /> be placed in the bag with the leaking item. <br /> -The following items were found in a tote labeled Universal Waste with accumulation start date of 10/10/18(IMG <br /> 4147): <br /> FA0001200 PR0538451 SCO01 02/07/2019 <br /> EHD 22-01 Rev.09/22/16 Page 4 of 8 Small Quantity Hazardous Waste Generator OIR <br />
