The Woi kplan must evaluate and propose additional offsite monitoring wells <br />' in shallow (first water), middle (30 to 60 feet bgs), and deep (over 125 feet bgs) water-beanng <br /> zones to verify groundwater pollution identified in Report Of Delineation Of Extent of <br /> Containlnation North Of The Site, dated 21 March 2000 (see Figure 3) Domestic wells are not <br />' adequate substitutes for properly constructed monitoring wells for delineation of groundwater <br /> pollution <br />' 8 By 1 February 2004, submit an Additional Site Characterization Report providing information <br /> from the implementation of the approved Workplan assessing the vertical and lateral extent of the <br /> groundwater pollution in all impacted aquifer zones <br /> I <br /> Final Remedial Actions <br />' 9 By 1 April 2004, submit a Final Corrective Action Plan (Plan) The Plan must include a <br /> description of the preferred remedial actions to address cleanup of the entire groundwater plume <br /> and source area soils The Plan may include optimization of the existing soil vapor extraction & <br /> air sparging, systems and include any additional remedial actions necessary to abate the effects of <br /> the Dischargers' release The Plan shall also include a schedule to implement all final remedial <br /> actions <br /> 10 Within 60 days of Executive Officer approval of the Final Corrective Action Plan, the Dischargers <br /> shall implement the approved remedial actions The Discharger shall notify staff a minimum of <br /> three working days prior to beginning fieldwork <br />' 11 Within 120 days of Executive Officer approval of the Final Corrective Action Plan, submit a <br /> report describing the status and results of the cleanup work (Cleanup Implementation Report) <br /> Report status of the installation of a cleanup system as complete, and if not, provide a schedule and <br /> proposed work plan for installing the remaining cleanup system(s), including a proposed <br /> monitoring plan <br /> 12 Upon startup of any remediation system(s), operate the remediation system(s) continuously, except <br /> for scheduled periodic maintenance or unpreventable equipment failure The Dischargers shall <br /> notify the Regional Board within 24 hours, if any unscheduled shutdown of the remediation <br /> system(s) lasts longer than 48 hours This notification shall include the cause of the shutdown and <br /> the corrective action taken (or proposed to be taken) to restart the system and assure the problem <br /> will not reoccur Any interruptions in the operation of the remediation system(s), other than for <br /> scheduled maintenance, emergencies, or equipment failure, without prior knowledge by Regional <br /> MBoard staff or without notifying the Regional Board within the specified time, is a violation of this <br /> Order <br /> I <br />
