No response plan was submitted. The response plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS upload the completed form, and submit for review by the EHD. Ensure a copy is available for inspection at any time. This is a minor violation. 107 CCR 2632(d), 2641(h), 2711(a) Plot plan/site map not submitted or failed to completely show where monitoring is performed. An accurate UST Monitoring Site Plan was not submitted. A site plan must be submitted identifying the locations where monitoring will be performed. The site plan must show the general layout of tanks and piping and clearly identify locations of the following equipment, if installed: 1) monitoring system control panels; 2) in -tank liquid level probes (if used for leak detection); 3) devices monitoring tank annular spaces or vault; 4) devices monitoring product piping; 5) devices monitoring fill piping; 6) devices monitoring vent piping; 7) devices monitoring vapor recovery piping; 8) devices monitoring vent/transition sumps; 9) devices monitoring under -dispenser containment; 10) line leak detectors; and 11) devices monitoring any other secondary containment areas. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/ and upload a copy of the UST Monitoring Site Plan. This is a minor violation. 118 CCR 2715(a)(1)(A), 2715(a)(2) Failure to submit the "Underground Storage Tank Statement of Understanding and Compliance Form". The Underground Storage Tank Statement of Understanding and Compliance Form was not found in CERS. This form must be uploaded to the California Environmental Reporting System (CERS). Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and upload the Underground Storage Tank Statement of Understanding and Compliance Form for this facility. This is a minor violation. 201 CCR 2712(b)(1-2) Failure to maintain monitoring, maintenance, and follow up action records. Maintenance and monitoring records for the last three years were not found on site. - Alarm logs -Overfill prevention test results These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain all missing maintenance and monitoring records for the last three years on site and submit copies to the EHD. This is a minor violation.
