Yosemite Avenue <br /> Manteca, CA <br /> San Joaquin County Environmental Health Department (EHD) has received a <br /> copy of the Central Valley Regional Water Quality Control Board letter to you <br /> dated November 10, 2003, requiring you to conduct a soil vapor survey on and <br /> around your property referenced above. Please be advised that San Joaquin <br /> County Ordinance Code requires that a well permit be obtained from EHD prior <br /> to the soil vapor survey or any other type of drilling or probing being performed. <br /> A contractor with a C-57 license must obtain the permit. There is a permit fee of <br /> $89 per parcel and staff inspection time is charged at $93 per hour. There is a <br /> master file record form (enclosed) that must be completed. The site owner name <br /> and address, the facility name and address, and the party responsible for paying <br /> any additional fees must be completed on the master file record form and the <br /> individual authorizing the billing must sign it. A fee of$279 (minimum 3 hours of <br /> time) must be submitted with the master file record form.
