♦ • • Well Permit Application Page 2 of 3 420 W.Yosemite Avenue, Manteca,CA October 5,2012 • MW 16AB northwest of the subject site on the off-site property located at 429 West Yosemite Avenue (APN# 219-311-06), directly north of the structure on-site, approximately 170 feet north from the edge of the property line along West Yosemite Avenue; and • MW 17A/B northwest of the subject site on the off-site property located at 531 West Yosemite Avenue(APN# 219-311-08), directly north of the structure on-site, and approximately 100 feet directly to the west of MW 16A/B and at the western edge of the property line. PROPOSED GROUNDWATER INVESTIGATION: The proposed work includes the installation of four dual-nested groundwater monitoring wells to depths of 30 and 70 feet each. Soil samples will be collected at 5-foot intervals from ground surface to terminal depth and logged for lithologic analysis only. The purpose of this investigation is to evaluate the extent of hydrocarbon impacts to groundwater in the down gradient direction, off-site to the northwest of the project property. It should be noted that AA&A provided for the installation of three dual-ne-ted groundwater monitoring wells MW14AB, MW15AB, and MW16AB in their Work Plan dated May 30, 2012. The CVRWQCB, In their directive letter approving the Work Plan, dated June 20, 2012, required the installation of one additional groundwater monitoring well MW 17AB. AA&A is submitting a application for a well permit for the installation of the four above-specified groundwater monitoring wells. Yosemite. • A copy of the completed permit application for MW 17AB (APN#219-311-08) located at 531 W. Yosemite. • A copy of the Site Location Map. • A copy of the Detail Site Map with the proposed groundwater monitoring well locations depicted. • A copy of the well construction diagram. • A copy of the San Joaquin County Parcel Maps. • A copy of AA&A's Off-site Groundwater Investigation Work Plan, dated May 30, 2012. • A copy of the corresponding CVRWQCB Workplan Approval Letter, Dated June 20, 2012. • A fully executed access agreement from the property owner of the property located at 429 West Yosemite Avenue(APN#219-311-06) granting AA&A permission to drill on the property to install MW 16AB. • A fully executed access agreement from the property owner of the property located at 531 West Yosemite Avenue(APN#219-311-08) granting AA&A permission to drill on the property to install MW 17AB.
