You have not provided all the required information to date. <br /> In a meeting held at PHS/EHD on December 23, 1999,you agreed to submit a reimbursement request to the <br /> Fund for the work already performed at the site within one week after the receipt of the a Letter of <br /> Commitment(LOC)from the Fund,and implement the work proposed in Wo?k XwAdderdi n Ah9wt 1999 <br /> dated August 10, 1999 prepared by AGE within one month after receiving reimbursement from the Fund. <br /> According to the staff at the Fund,a LOC dated February 11,2000 was mailed to you but you did not submit <br /> the reimbursement request until March 7,2000.When you failed to provide all the required information to the <br /> Fund,your reimbursement request was suspended. The proposed work has not commenced to date. <br /> Since the approval of the work plan on July 7,1999,and the work plan addendum on August 25, 1999, <br /> PHS/EHD had twice agreed to your request to extend the implementation of the proposed work because of <br /> your claimed financial difficulties. When you failed to meet those deadlines,PHS/EHD again agreed to <br /> postpone implementation of the proposed work until you received the reimbursement from the Fund with the <br /> understanding that you are actively pursuing clean-up of the underground storage tank released petroleum <br /> contamination at the site in accordance with le 23,Division 3,Chapter 16 California Code of Regulations <br /> and requesting reimbursement from the Fund.
