• Well Permit Application Page 2 of 2 <br /> 420 W.Yosemite Avenue,Manteca,CA <br /> January 22,2013 <br /> Pursuant to your email dated 10-17-12 (copy attached), we are providing the additional fees required for the subject <br /> well permit application revision/resubmittal as well as revised diagrams and plans. <br /> Attached are the following for the permit application resubmittal: <br /> 1. A copy of the revised well permit applications. We completed new applications which have been signed by <br /> the driller. These new applications will replace the ones that were previously approved by your office. <br /> 2. A copy of the revised Site Plan depicting the locations of the 8 split wells. <br /> 3. A copy of the revised well construction diagrams. We now have two well construction diagrams;one for the <br /> shallow wells and one for the deeper wells. <br /> 4. A copy of your 10-17-12 email regarding additional permit fees and resubmittal requirements. <br /> 5. For reference,a copy of the approved permit applications for the dual-nested wells, issued 10-25-12 (Permits <br /> SR-65997 through SR-65999). <br /> We understand the additional permit fees are as follows: <br /> $125 fee each additional paired well to split: $125.00 x 4=$500.00 <br /> Attached is a check in the amount of$500.00 to cover the additional permit fees. <br /> Please let me know if you need anything further.
