The monitoring plans are not current and/or not approved by the EHD. -The plans for all three tanks left blank"UDC monitoring stops flow of product at the dispenser". The monitoring plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, and submit for review by the EHD. This is a repeat violation, Class II. 113 CCR 2638(c), 2638(d) Failure to submit"Monitoring System Certification Form"within 30 days of completion of the test. Annual monitoring system certification, leak detector testing, and spill container testing was performed on 5/25/18 and a test report was submitted on 7/17/18, one month late. A copy of the test report must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. This is a minor violation. 115 CCR 2715(a)(1)(B), 2715(a)(3)"Designated Underground Storage Tank Operator Identification Form" not submitted within 30 days. Zane Nimmo is performing designated operator inspections at this facility. He is not on the submitted list and notification was not provided to the EHD within 30 days of the change. Any changes shall be reported to the EHD within 30 days. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility. This is a minor violation. 204 CCR 2712(1) Current monitoring plan approved by the EHD not found on site. An approved copy of the monitoring plans were not available for inspection. The plans on site were dated 2014. Updates were made to the monitoring plans in 2018. A copy of these plans shall be accessible on site at all times. Immediately locate and retain a current copy on site. This is a minor violation. 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. A designated operator inspection for the following months was performed more than 30 days: -Conducted 1/20/19, next conducted 2/21/19 (32 days) The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection.
