SAN J O A Q U I N Environmental Health Department <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Dollar Tree#02983 2691 N Tracy Blvd, Tracy May 28 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS 1,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> stacks of product boxes was an opening that leads to the store's sales shelves which contained various cleaning <br /> products. Some of the cleaning products were seen to have fallen over onto the product boxes, and potentially could <br /> fall onto the ground causing further spills. (IMG 6812-6) <br /> 11. The following containers were observed on shelves throughout the store's back storage area(IMG 6817,6820-4, <br /> 6831-3): <br /> a.A partially filled 40 fl oz bottle of LA's Totally Awesome Window Cleaner.This cleaner was next to a container of <br /> LA's Totally Awesome Bleach,which was marked with an orange sticker on the item's barcode,which according to <br /> Ryan was Dollar Tree's way of marking items designated to clean the store. Several other cleaning chemical <br /> containers marked with the orange store-use stickers were observed on a nearby shelf. Please note that all other <br /> previously mentioned chemical items/damages were not marked with an orange store-use sticker. <br /> b.A partially filled 20 fl oz bottle of LA's Totally Awesome All Purpose Cleaner, not marked with an orange <br /> store-use sticker. <br /> c.A partially filled 22 fl oz bottle of Spic and Span Antibacterial Spray Cleaner, not marked with an orange <br /> store-use sticker. <br /> There was no used absorbent or spill clean-up material found to account for the missing liquids from the partially <br /> empty items mentioned above. Ryan did not know what happened to the missing liquids. <br /> 12.A white powdery substance was found on several sales shelves which contained bottles of LA's Totally Awesome <br /> All Purpose Cleaner and Window Clean for sale(IMG 6835-7, 6841).The white substance appeared to Comet Bleach <br /> cleaning powder. <br /> 13. Historic spillage and staining was found on the sales shelf which contained containers of LA's Totally Awesome <br /> Bleach and The Home Store Bleach for sale(IMG 6838-9, 6842-3). <br /> 14. Historic spillage and staining, blue in color,was found on the sales shelf which contained aerosol cans of The <br /> Home Store Foaming Super Oxygen Bathroom Cleaner and Heavy Foam Shower Cleaner(IMG 6840). <br /> 15. Historic spillage and staining of nail polish was observed on the floor in front of the sales shelves containing nail <br /> polish(IMG 6844-5). <br /> Ryan provided a list of current employees and the date they last received hazardous waste training.
