SA N 1 O A O I I N Environmental Health Department Small Quantity Hazardous Waste Generator Inspection Report Facility Name: Facility Address: Date: Dollar Tree#02983 2691 N Tracy Blvd, Tracy May 28. 2019 SUMMARY OF VIOLATIONS (CLASS I,CLASS II,or MINOR-Notice to Comply) Item# Remarks 301 CCR 66262.34(d)(2) Facility not maintained to minimize the release of a hazardous waste. 1.The following was found in an open cardboard box on the floor beneath the store's baskets containing"go-back"and damaged items. (IMG 6736-8, 6741): a. A bottle of glue leaking out onto the bottom of the cardboard box.The spilled glue appeared to be mixed with spilled blue paint. Ryan did not know how long the leakage had been there, but believed the items would ultimately be processed as HW.When Ryan scanned the glue,the hand-held scanner stated the glue should be placed in an STOU HW tote. 2.The following items were found in or next to the second of two open cardboard boxes, which were sitting on a pallet that was located on the floor next to the cardboard bailer. A sign posted on the wall above the items read"Damages No Trash,"and was marked with an arrow pointing down toward the items(IMG 6791-6799): a. Five open bottles, some of which were partially empty, of White Rain Mens Body Wash.The body wash leaked out of the containers, and onto the inside of the box.The leak soaked through the bottom of the box and onto another cardboard box, and appeared to have leaked onto and under the pallet onto the floor. 3.The following items were found underneath the pallet on the floor(IMG 6800-2, 6807-9): a.An open partially filled 3 fl oz bottle of Aleene's Tacky Glue,which was missing its cap and had dry glue leaking out of the bottle. 4.What appeared to be a dried spill of an unknown blue liquid and staining from another spill,were found in an aisle in the store's back storage area, next to a shelf containing stacks of product boxes.A cap that appeared to be from a dish soap container was found on the blue liquid spill.The inside of the cap contained the blue liquid. Above the stacks of product boxes was an opening that leads to the store's sales shelves which contained various cleaning products. Some of the cleaning products were seen to have fallen over onto the product boxes, and potentially could fall onto the ground causing further spills. (IMG 6812-6) 5. Historic spillage and staining of nail polish was observed on the floor in front of the sales shelves containing nail polish(IMG 6844-5). Facilities shall be maintained and operated to minimize the possibility of a fire,explosion, or release of hazardous waste to air, soil, or surface water which could threaten human health or the environment. Immediately clean the spillage and staining, and manage according to Title 22 hazardous waste regulations. Submit a statement and supporting documentation explaining how this waste was managed.
