Financial responsibility documents have not been submitted to the EHD. Current financial responsibility documents were not found on site and are required to be submitted annually. Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, and upload the required documents. This is a repeat violation, Class II. 105 CCR 2632(d)(1), 2634(d) Failed to have an approved UST monitoring plan. The monitoring plan is not current and/or not approved by the EHD. The person having responsibility is listed as Ted Shackelford, however he is no longer working for the facility. All tanks list YES under automatic tank gauging, and YES under other monitoring(daily volume checks). Furthermore, the monitoring plan for all tanks state yes under record keeping for Tank integrity testing, SIR testing,Tank gauging ,ATG testing, and corrosion protection logs, however these records were not found on site,when the maintenance supervisor was asked if for the records, he was not able to provide them to the inspector. The monitoring plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, and submit for review by the EHD. This is a repeat violation, Class ll. 106 CCR 2632(d)(2), 2634(e) Failed to have an approved UST response plan. The response plan is not current and/or not approved by the EHD. In the response plan,Ted Shackelford is listed as a contact person, however he is no longer working for the facility. The response plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, upload the completed form, and submit for review by the EHD. Ensure a copy is available for inspection at any time. This is a repeat violation, Class ll.
