Maintenance and monitoring records for the last three years were not found on site. Maintenance records for L2-91 fill fuel alarm on 11/30/18, L8-87 STP fuel alarm on 12/17/18 and L6-Red diesel fuel alarm on 1/11/19 were not found on site. the monitoring plan for all tanks state yes under record keeping for Tank integrity testing, SIR testing, Tank gauging , ATG testing, and corrosion protection logs, however these records were not found on site,when the maintenance supervisor was asked if for the records, he was not able to provide them to the inspector. These records shall be maintained on site for at least three years. Monitoring records include: (1)date and time of all monitoring or sampling; (2)monitoring equipment calibration and maintenance records; (3)results of any visual observations; (4)results of sample analysis performed a lab or in the field; (5)logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6)results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit copies to the EHD. This is a repeat violation, Class II. 202 CCR 2715(c)(3), 2715(c)(4)Failure to maintain the"Facility Employee Training Certificate"on-site(after October 1, 2018). "Facility Employee Training Certificate"dated 12/10/19 was found on site stating that employee, Ronnie Ignacio was trained on 12/10/19. Ronnie Ignacio'date of assuming responsibility is stated as 11/20/19 however the initial training date was stated to be 12/10/19 both of which dates are in the future, ensure the DO is writing correct dates under the"Facility Employee Training Certificate".All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018.The"Facility Employee Training Certificate"shall contain accurate information for all employees trained. Update the"Facility Employee Training Certificate"to contain the accurate information and submit a copy of the updated form to the EHD. This is a minor violation. 204 CCR 2712(i) Current monitoring plan approved by the EHD not found on site. An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a repeat violation, Class II. 205 CCR 2712(i), 2634(e) Failure to submit a current UST Response Plan and have it available onsite. An approved copy of the response plans were not available for inspection. A copy of these plans shall be accessible on site at all times. Immediately locate and retain a copy on site.
