Time In: 12:30 pm <br /> Time Out: 1:00 om <br /> Food Program Complaint Inspection Report <br /> Name of Facility: PAMPANGAS BAKERY RESTAURANT Date: 07/27/2020 <br /> Address: 1619 E HAMMER LN, STOCKTON 95210 <br /> Owner/Operator: MALLARI, DANILO M Telephone: (209)956-1500 <br /> Program Element: 1600-FOOD PROGRAM Complaint#: C00052308 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> COMPLAINANT ALLEGES FACILITY HAS ROACHES. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> #23 Rodents, Insects or Animals Inside Facility <br /> OBSERVATIONS:Many dead cockroaches were observed under the water heater. Few live cockroaches observed in the <br /> kitchen. Remove these dead cockroaches today and increase pest control measure. Reinspection on or after August 10, <br /> 2020. <br /> CALCODE DESCRIPTION:Each food facility shall be kept free of vermin:rodents(rats,mice), cockroaches,flies.(114259.1, 114259.4, <br /> 114259.5) <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> Complainant inspection conducted this date,the followings were noted: <br /> Ms. Dinna Mallari, manager, stated the facility was closed to spray for cockroach on 7/20/2020. <br /> Few live baby cockroaches were observed during inspection.
