SAN J O A Q U I N Environmental Health Department <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Smart& Final#403 1 147 N Wilson Way, Stockton October 31, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> was found: <br /> a. not labeled with the store name, city,and state; <br /> b. missing the year on the accumulation start date; <br /> c. not labeled with the physical state; and <br /> c. open with its lid not latched,while waste was not actively being added or removed from the tote(IMG <br /> 9166-8). <br /> 6.A tote labeled"TBD"was open, missing it's lid and both lid latches, and contained an open bag of Roma powder <br /> laundry detergent, an open bag of Tide powder laundry detergent, and an unknown yellow brown liquid at the <br /> bottom of the tote. Each open bag of laundry detergent was also bagged in a Stericycle bag labeled"One Waste <br /> Per Bag"and marked with a date of"10/10"(IMG 9177-83). According to Almadelia Santiago, Receiver, potentially <br /> hazardous waste is placed in the To Be Determined(TBD)tote temporarily, until she can determine if the waste is <br /> hazardous waste.Almadelia stated the bags of laundry detergent were placed in the TBD tote on 10/10/2019. <br /> Thomas Champion, Sr Assistant Manager, stated it appeared that the bags of laundry detergent were left in the <br /> TBD tote because all of the other hazardous waste totes were full and could no longer fit more waste.The TBD tote <br /> was missing the words"Hazardous Waste", name and address of generator, hazardous properties, physical state, <br /> composition(contents), accumulation start date. <br /> 7.The hazardous waste management training posters were found posted on the shelving and wall behind the <br /> hazardous waste totes,and were not easily visible to read(IMG 9154, 9196). <br /> REGULATION GUIDANCE: The business owner must ensure that all employees are thoroughly familiar with <br /> proper waste handling and emergency procedures, relevant to their responsibilities during normal facility operations <br /> and emergencies. <br /> CORRECTIVE ACTION: Immediately train the employees.
