SAN J O A Q U I N Environmental Health Department <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Smart& Final#403 1 147 N Wilson Way, Stockton October 31, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 106 CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br /> OBSERVATION: At the time of inspection, it could not be demonstrated(not necessarily documented)that <br /> employees who handle hazardous waste were properly trained, based on the following observations: <br /> The Hazardous Waste Accumulation Center Inspection documents, found in the hazardous waste storage area <br /> and dated 9/8- 11/4/2019, indicated that the hazardous waste totes(containers/bins)were in good condition and <br /> not cracked; properly labeled and include the accumulation start date, physical state, hazardous properties, <br /> contents/composition, and store information(name&address);closed,except when it is necessary to add or <br /> remove the waste; and that the training manual and posters were visible(see photos IMG 9194-5). However,the <br /> following observations were made in the hazardous waste storage area: <br /> 1.A Corrosive Bases(CB)Hazardous Waste tote, containing bagged containers of bleach,powder and liquid <br /> laundry detergents, and floor cleaners,was found: <br /> a. not labeled with the store name, city, and state; <br /> b. labeled inaccurately for hazardous properties and contents/composition, since according to the training <br /> manual and posters, bleach is an Oxidizer; <br /> c. labeled inaccurately for contents/composition,since bleach was not written on the label under Contents; and <br /> c.cracked and broken, causing the tote to be open(IMG 9155-7). <br /> 2.An Oxidizers(OX) Hazardous Waste tote, containing at least five containers of bleach,was found: <br /> a. not labeled with the store name, city,and state; and <br /> b. missing both of its lid latches,causing the tote to not be closed properly(IMG 9158-9159). <br /> 3.A Corrosive Acids(CA)Hazardous Waste tote, containing powder laundry detergent and Comet with bleach <br /> powder,was found: <br /> a. not labeled with the store name, city,and state; <br /> b. not labeled with the accumulation start date; <br /> c. inaccurately labeled for physical state, since liquid was written on the label, although there was no liquid <br /> found in the tote; <br /> d. inaccurately labeled for hazardous properties, since according to the training manual and posters, detergents <br /> are either Corrosive Bases or Toxic&State Regulated Hazardous Waste, and waste containing bleach are <br /> Oxidizers; <br /> e. inaccurately labeled for contents/composition,since soap scum removers and dry dishwasher detergents <br /> were marked on the label under Contents, although neither one of those wastes were found in the tote; and <br /> f. cracked and broken, and missing one of its lid latches, causing the tote to be open and not closed properly <br /> (IMG 9171-6). <br /> 4.An Aerosols, Flammables and Combustibles(AF) Hazardous waste tote, containing aerosol cans,was found: <br /> a. not labeled with the store name, city, and state; <br /> b.missing the year on the accumulation start date; <br /> c. not marked with the appropriate physical state; and <br /> c. open with its lid not latched,while waste was not actively being added or removed from the tote(IMG <br /> 9160-5). <br /> 5.A Toxics&State Regulated(TX) Hazardous Waste tote, containing soaps and Gain Carpet Deodorizing powder, <br /> FA0001579 PR0539378 SC001 10/31/2019 <br /> EHD 22-01 Rev.9/20/2019 Page 5 Of 15 Small Quantity Hazardous Waste Generator OIR <br /> 1868 E.
