REGULATORY GUIDANCE:Within 48 hours of the completion of the designated UST operator visual inspection, the designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 48 hours of being provided a signed copy of the report,the owner or operator shall provide a description of each corrective action taken or to be taken and shall sign and date the report, acknowledging the identified compliance issues. CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed and signed within the required time frame in the future. 2)OBSERVATION:The designated operator failed to check and note the correct test dates for the secondary containment and monitoring system certification testing on the designated operator inspection reports. Routine secondary containment testing was performed on 10/17/2016 and 10/18/2019. Routine monitoring system certification testing was performed on 7/1/2019. REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. 3)OBSERVATION:The designated operator failed to attach the alarm history on the 7/29/2019 designated operator inspection report. REGULATION GUIDANCE: During the inspection,the designated operator shall review the alarm history since the previous inspection, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the inspection report. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. 4)OBSERVATION:The designated operator failed to check and note all required items on the visual inspection reports.The incorrect and missing information included the following from the inspection reports: -The"N/NA"in sections VII through XI were not fully explained in section III of the report. -The designated operator documented visual inspections of the 87 and 91 sump but left the question regarding sump sensor locations blank. -The designated operator documented visual inspections of the UDCs but left the question regarding UDC sensor FA0001060 PR0231704 SCO01 07/02/2020 EHD 23-01 Rev.9/20/2019 Page 6 of 8 Underground Storage Tank Program OIR 1868 E.
