Financial responsibility documents have not been submitted to the EHD. The financial responsibility documents submitted on CERS are dated 1/11/18. Current financial responsibility documents are required to be submitted annually. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, and upload the required documents. This is a minor violation. 104 CCR 2711 UST operating permit application for facility and tank information not submitted or current. UST Tank Information forms for the 87 and diesel tanks are not current in CERS. -The diesel tank form incorrectly states the riser is fiberglass. -The 87 tank form incorrectly states the vent lines are double-walled. Any change of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, update the required information, and submit for review by the EHD. This is a Class II violation. 105 CCR 2632(d)(1), 2634(d) Failed to have an approved UST monitoring plan. The monitoring plans are not current and/or not approved by the EHD. -The diesel monitoring plan incorrectly states automatic tank gauging is being conducted. -The diesel monitoring plan incorrectly states the overfill prevention flappers are 6150. The accurate model is 61 SO. -The diesel monitoring plan incorrectly states ATG test results are maintained on site and maintenance and calibration records are not maintained. -Both monitoring plans incorrectly list the monitoring panel model as a TLS350 or just a 350. The monitoring panel on site is a TLS-350 Plus. The monitoring plans must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, and submit for review by the EHD. This is a minor violation. 107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. An accurate UST Monitoring Site Plan was not submitted. Three site maps were uploaded in the most recent CERS submittal, but none contains complete, accurate information. A site plan must be submitted identifying the locations where monitoring will be performed. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/and upload a copy of the complete UST Monitoring Site Plan. Outdated previously submitted documents should be discarded from the submittal if they are no longer being used to meet the UST requirements. This is a repeat violation, Class II. 110 CCR 2637(e), 2637(f) Failed to submit"Secondary Containment Testing Report Form"within 30 days after the test. Secondary containment testing was performed on August 29, 2018 and a test report was submitted on October 29, 2018, one month late. A copy of the test report must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner.
