Environmental Health Department <br />AMENDED <br />12/9/2019 <br />Small Quantity Hazardous Waste Generator Inspection Report <br />Date: <br />December 04, 2019 <br /> Facility Address: <br /> 2477 Naglee Rd, Tracy <br /> Facility Name: <br /> PetSmart #1164 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 106 CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br />OBSERVATION: At the time of inspection, it could not be demonstrated (not necessarily documented) that <br />employees who handle hazardous waste were properly trained, based on the following observations: <br />1. The following waste was observed in a trash can in the Petsmart Grooming salon's back storage area: <br /> A. A 1-gallon container of HydroSurge Face Shampoo was observed open and containing a pourable amount of <br />shampoo. <br /> B. A 1-gallon container of Hempz Petz Conditioner was observed open and leaking out a pourable amount of <br />conditioner. <br />According to the Petsmart Potentially Hazardous Waste Management Poster in the hazardous waste storage area, <br />pet shampoo and conditioner are classified as State Regulated (ST) Hazardous Waste. <br />Tawnya Jeffcoat, Merchandise Inventory Leader, removed the containers of shampoo and conditioner and placed <br />them on a desk next to the trash can. Lyndi Shwinn, Grooming Manager, was informed of the containers of <br />shampoo and conditioner found in the trash. <br />2. The following hazardous waste totes, observed in the hazardous waste storage area, were missing the lid clamp <br />closures causing the lids to be insecure and not properly closed, and were missing hazardous waste label <br />information: <br /> A. A Toxic (TX) Hazardous Waste tote storing three plastic syringes and five bottles labeled as Enrofloxacin and <br />Baytril medication. The syringes and bottles appeared empty. According to Tawnya Jeffcoat, Merchandise Inventory <br />Leader, the medication and syringes were used to treat the facility's animals. <br /> The TX hazardous waste label was missing the generator name, physical state, composition (contents), and <br />had an inaccurate accumulation start date of 4/20/2018. The facility had hazardous waste pick-ups after <br />4/20/2018, on 8/1/2018, 1/23/2019, 7/3/2019. <br /> B. A State Regulated (ST) Hazardous Waste tote storing a large plastic bag containing at least 50 non-empty <br />used plastic containers total of Davis Veterinary Products Ear Med Pro Cleanse (12 fl. oz.), Fetch Innovation <br />Unleashed Eye Wash (8 fl. oz.), and Hydrosurge shampoo and conditioner, all for dogs and/or cats. According to <br />Tawnya, the Grooming salon used these products and placed the used containers into the hazardous waste tote. <br /> The ST hazardous waste label had an inaccurate accumulation start date of 4/20/2018. <br /> C. A Corrosive Bases (CB) Hazardous Waste tote storing an API PondCare Pond Master Test Kit containing <br />two bottles of Ammonia Test Solution, one bottle of Nitrite Test Solution, two bottles of Phosphate Test Solution, <br />and one bottle of Wide Range pH Test Solution, all of which were 37 ml each in size. <br /> The CB hazardous waste label was missing the generator name, physical state, composition (contents), and <br />had an inaccurate accumulation start date of 4/20/2018. <br />3.
