Environmental Health Department <br />AMENDED <br />12/9/2019 <br />Small Quantity Hazardous Waste Generator Inspection Report <br />Date: <br />December 04, 2019 <br /> Facility Address: <br /> 2477 Naglee Rd, Tracy <br /> Facility Name: <br /> PetSmart #1164 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br />shampoos and conditioners was observed in a back storage area of the Petsmart Grooming salon. Lyndie Shwinn, <br />Grooming Manager, did not know how long ago the containers of used shampoo and conditioners had began to <br />accumulate in the cardboard box. According to the PetSmart Potentially Hazardous Waste Management poster in <br />the hazardous waste storage area, pet shampoo and conditioner are classified as State Regulated (ST) Hazardous <br />Waste. <br />4. At the time of inspection, it could not be demonstrated that the facility's hazardous waste containers are <br />adequately inspected weekly, based on the following observations: <br /> A. The most recent completed waste storage area weekly inspection checklist, found in the Petsmart Hazardous <br />Material Disposal Process Binder in the hazardous waste storage area, was dated 12/10/2018. According to Marie <br />Balingit, Customer Engagement Leader, and Tawnya Jeffcoat, Merchandise Inventory Leader, John Thorpe is the <br />primary employee who manages the hazardous waste storage area and conducts the weekly inspections. <br />According to the Petsmart Retail Hazardous Products Disposal Program Training document in the binder, the <br />storage manager is to complete the waste storage area weekly inspection checklists and maintain the completed <br />checklists in the binder for the past 3 years. Completed inspection checklists dated after 12/10/2018 were not <br />found at the time of inspection. <br /> B. The waste storage area weekly inspection checklists dated 12/10/2018, 7/12/2018, and 4/21/2018 indicated <br />that the hazardous waste containers were not properly closed, without stating/indicating what corrective actions <br />were implemented to correct the issue. <br />5. The following potential incompatible wastes were observed stored in way that is not in compliance with Title 22 <br />CCR: <br />A Corrosive Bases (CB) Hazardous Waste tote storing an API PondCare Pond Master Test Kit containing two <br />bottles of Ammonia Test Solution, one bottle of Nitrite Test Solution, two bottles of Phosphate Test Solution, and <br />one bottle of Wide Range pH Test Solution, all of which were 37 ml each in size. <br />REGULATION GUIDANCE: The business owner must ensure that all employees are thoroughly familiar with <br />proper waste handling and emergency procedures, relevant to their responsibilities during normal facility operations <br />and emergencies. <br />CORRECTIVE ACTION: Immediately train/re-train the employees.
