Environmental Health Department <br />Small Quantity Hazardous Waste Generator Inspection Report <br />Date: <br />November 20, 2019 <br /> Facility Address: <br /> 10520 Trinity Pkwy, Stockton <br /> Facility Name: <br /> PetSmart #1188 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 111 CCR 66262.42(a)(c)(d) Failed to comply with uniform hazardous waste manifest exception requirements. <br />OBSERVATION: The facility did not have exception reports on site for the following Uniform Hazardous Waste <br />Manifests (UHWM): <br />- 013120049FLE (7/17/2019) (generator copy also not found on site) <br />- 012956359FLE (1/24/2019) (see photo IMG 9485) <br />- 010740496FLE (8/8/2019) (IMG 9486) <br />- 017622103JJK (12/20/2017) (generator copy also not found on site) <br />- 016937182JJK (8/10/2017) (generator copy also not found on site) <br />REGULATION GUIDANCE: <br />(a) A generator who does not receive a copy of the manifest with the handwritten signature of the owner or operator <br />of the designated facility within 35 days of the date the waste was accepted by the initial transporter shall contact <br />the transporter and/or the owner or operator of the designated facility to determine the status of the hazardous <br />waste. <br />(c) A hazardous waste generator who does not receive a copy of the manifest with the handwritten signature of the <br />owner or operator of the facility to which the generator's waste is submitted, within 60 days from the date that the <br />hazardous waste was accepted by the initial transporter, shall submit to the department a legible copy of the <br />manifest, with some indication that the generator has not received confirmation of delivery. <br />(d) Generators shall submit the information to the department at: DTSC Report Repository, Generator Information <br />Services Section, P.O. Box 806, Sacramento, CA 95812-0806 <br />CORRECTIVE ACTION: Locate a copy of the destination facility signed manifests listed above and send a copy to <br />the San Joaquin County Environmental Health Department (EHD). If the destination facility signed manifests <br />cannot be found then send to DTSC a copy of each generator signed manifest and a statement that the designated <br />facility signed copy was not received by the generator.
