0 HAZARD*ALYSIS PAG.• <br /> FEATURE DRILLING ANALYZED BY/DATE REVIEWED BY/DATE <br /> PRINCIPLE POTENTIAL RECOMMENDED <br /> STEPS HAZARDS CONTROLS <br /> kept out of the work area and in an upwind location. <br /> Cross contamination No Exclusion Zone activities are associated with this risk. <br /> Electrical shock All electrical circuits will be deengergized and locked out. <br /> Electrical Ground fault circuit interrupters, inspect extension cords, hand <br /> tools/electrocution tool inspection, lockout-tagout procedure. <br /> Elevated Work Positive fall protection provided with full body harness worn by all <br /> workers in a potential free fall position. <br /> Falling objects Hardhat, stay alert and clear of materials suspended overhead, <br /> steel-toed boots. <br /> Heat stress/Cold Recognize warning signs. <br /> Stress <br /> Heavy Equipment Constant employee awareness; visual/verbal contact with operators. <br /> Operation <br /> Heavy lifting Use proper lifting techniques. Lifts greater than 60 lbs require <br /> assistance or mechanical equipments size-up the lift. Recommend <br /> wearing a back support if possible. <br /> Hoisting equipment All hoisting equipment shall be capable of passing a performance <br /> (operating) test prior to being placed into service. <br /> At no time shal the hoisting equipment be loaded in excess of the <br /> manufacturers rating. <br /> While hoisting equipment is in operation, the operator shall not <br /> perform any other work and he/she shall not leave his/her position <br /> at the controls until the load has been safely landed or returned to <br /> the ground. <br /> A standard signal system shall be used on all hoisting equipment. <br /> Lifting Instruction in proper lifting techniques; instruction to not sift <br /> large or heavy objects without assistance. <br /> Moving equipment Signal person will assist in posistioning equipment. <br /> Signal person will wear a reflective vest for high visibility. <br /> Noise Hearing protection is mandatory above 85 dBA. <br /> Pinch points Keep hands and feet clear of moving/suspended materials and <br /> equipment. <br /> Stay alert at all times. <br /> Beware of contact points. <br />
