• HAZARANALYSIS PAC <br /> FEATURE BACKFILL AND COMPACT ANALYZED BY/DATE REVIEWED BY/DATE <br /> PRINCIPLE POTENTIAL RECOMMENDED <br /> STEPS HAZARDS CONTROLS <br /> Backfilling and Dump truck operations Dump truck bodies shall be fully lowered or blocked when maintenance <br /> Compaction is being performed or when not in use. <br /> Dump trucks will have back up alarms. <br /> A signal person will be used when the pint of operation is not in <br /> full view of the vehicle, machine or equipment operator; vehciles <br /> are backed more than 100 ft; terrain is hazardous= or 2 or more <br /> vehicles are backing in the same area. <br /> Operators of dump trucks will leave the Cab while being loaded. <br /> Dump trucks will not be loaded in a manner that obscures the <br />! operator's view ahead or to either side of that interferes wtih the <br /> afe operation of the vehicle. <br /> The load on every truck will be distributed, checkd, tied down, or <br /> secured. <br /> Loads will be covered when there is a hazard of flying/falling dirt, <br /> rock, debris, or material. <br /> All dump trucks will be equipped with a hoding device for prevent <br /> accidental lowering of the body. <br /> All hoist levers will be secured to prevent accidental starting or <br /> tripping of the mechanism. <br /> Trip handles for tailgates will be arranged to keep the operator in <br /> the clear. <br /> Excavation and Follow policy procedures for safe trench excavation. <br /> trenching <br /> Falls Lanyards, lifelines, and ladder/scaffolding safety. <br /> Heavy Equipment Constant employee awareness; visual/verbal contact with operators. <br /> Operation <br /> Moving equipment Signal person will assist in posistioning equipment. <br /> Signal person will wear a reflective vest for high visibility. <br /> Noise Hearing protection is mandatory above 85 dBA. <br /> Open excavations Excavations will be backfilled as soon as possible. <br /> Pinch points Keep hands and feet clear of moving/suspended materials and <br /> equipment. <br /> Stay alert at all times. <br /> Beware of contact points. <br /> Slip, trip, and fall Determine best access route before transporting equipment. <br /> hazards Good housekeeping, keep work area picked up and clean as feasible. <br />
