<br /> The Employee Hazardous Waste Training Log observed at the time of inspection showed that facility employees
<br /> have not been trained since January, February, and August of 2017(IMG 3062).According to the store manager,
<br /> Francisco Valle,the employees listed on the training log are former employees that no longer work at the facility.
<br /> Francisco stated that he does not have training records for the current employees.When asked if current
<br /> employees have received training, Francisco stated that he received a brief store manager training when he was
<br /> promoted about one year ago,which covered hazardous waste management procedures, however the other
<br /> employees have not received training other than the information he has provided to them based on his own training.
<br /> Based on the types of violations observed,the facility employees are not trained on proper hazardous waste
<br /> handling or the training provided has been inadequate.
<br /> The following observations were made in the store's back storage room at the time of inspection:
<br /> -The hazardous waste (HW)totes for Corrosive Bases, Toxic and State Regulated Waste, and Univeral &E-Waste
<br /> were found open due to cracked and broken lids(see photos: IMG 2954-2956, 3099, 3101-3102).
<br /> -The most recent Weekly Inspection Logs found on site were completed by Francisco Valle,the store manager,
<br /> and were dated 5/24/2018 and 5/31/2018(IMG 3080). Both of these weekly logs marked "No"for the questions"Are
<br /> all containers in good condition"and"Are all containers marked with an accumulation start date,"however there
<br /> were no repairs or remedial actions noted on the logs.
<br /> -All six of the hazardous waste totes observed had an accumulation start date of 8/10/2017 and lacked the full
<br /> name of the generator(IMG 2955, 2957, 2965, 2988, 3021, 3049, 3056).There was one tote for Corrosive Bases;
<br /> one for Aerosols, Flammables, &Combustibles; one for Corrosive Acids; one for Oxidizers, one for Toxic&State
<br /> Regulated Waste; and one for Universal &E-Waste.The store manager verified the most recent hazardous waste
<br /> pick up was on 8/17/18, per manifest 017403264JJK.
<br /> -A Clairol creme permanent developer, an AGE Beautiful creme developer, and a Wella Colortango developer
<br /> oxidant were found in the Corrosive Bases HW tote, although according to a label on the Oxidizers HW tote, all
<br /> volumes of developer are to be placed in the Oxidizers HW tote(IMG 2957, 2960-2963, and 3049).
<br /> -A Brights semi-permanent creme hair color was found in the Corrosive Bases HW tote, although according to a
<br /> label on the Toxic&State Regulated HW tote, semi-permanent hair color is to be placed in the Toxic&State
<br /> Regulated HW tote(IMG 2964 and 3021).
<br /> -The following was found in the Corrosive Acids HW tote(IMG 2965):
<br /> -A bottle of Gigi Post Epilation Lotion skin moisturizer, although according to a label on the Toxic&State
<br /> Regulated HW tote, body lotion is to be placed in the Toxic&State Regulated HW tote(IMG 2969).
<br /> -A bottle of Gigi Post Wax Concealer skin foundation, although according to a label on the Toxic&State Regulated
<br /> HW tote,foundations are to be placed in the Toxic&State Regulated HW tote(IMG 2970).
<br /> -Three aerosol cans of AGE Beautiful Root Touch-Up temporary hair color, although according to a label on the
<br /> Aerosols, Flammables, &Combustibles HW tote, any aerosols cans are to be placed in the Aerosols, Flammables,
<br /> &Combustibles HW tote(IMG 2977-2979, 2988).
<br /> -Spillage of what appeared to be pink Gigi Creme Wax for sensitive skin was found at the bottom of the tote,
<br /> however absorbent material used for the spill clean-up was not found in the tote(IMG 2966-2968, 2987).According
<br /> to the Sally Beauty Holdings, Inc. Hazardous Waste Training Guide,the item spilled and used absorbent and gloves
<br /> are to be bagged and placed in the correct hazardous waste bin, and logged in the hazardous waste binder(IMG
