<br /> -The following was found in the Aerosols, Flammables, &Combustibles HW tote(IMG 2988):
<br /> - Ion Color Brilliance Permanent Brights creme hair color(IMG 2990), although according to a label on the
<br /> Corrosive Bases HW tote, cream permanent hair color is to be placed in the Corrosive Bases HW tote(IMG 2957).
<br /> -Spillage of an unknown liquid was found at the bottom of the tote, however absorbent material used for the spill
<br /> clean-up was not found in the tote(IMG 3014-3015).According to the Sally Beauty Holdings, Inc. Hazardous Waste
<br /> Training Guide,the item spilled and used absorbent and gloves are to be bagged and placed in the correct
<br /> hazardous waste bin, and logged in the hazardous waste binder(IMG 3063, 3070). Francisco Valle,the store
<br /> manager, cleaned the spillage using absorbent, placed the absorbent in a bag and placed the bag into the tote(IMG
<br /> 3020).
<br /> -The following was found in the Toxic&State Regulated HW tote(IMG 3021):
<br /> -A bag containing what appeared to be leakage from a bottle Ion Reconstructor for weak damaged hair(IMG 3042)
<br /> lacked absorbent.According to the Sally's Beauty Guidelines for Accumulating Hazardous Waste poster,"if a
<br /> container is leaking,then absorbent must be placed in the bag" (IMG 2959).The store manager applied absorbent
<br /> to the leaking bottle, and double bagged the leakage at the time of inspection.
<br /> -A bottle of BTZ Sugar Rush Hair Perfume(IMG 3044-3045), although the label on the bottle indicated the item
<br /> was flammable. The store manager placed the item into the Aerosols, Flammables, and Combustibles HW tote at
<br /> the time of inspection.
<br /> -Spillage of what appeared to be pink and yellow wax was found at the bottom of the tote, however absorbent
<br /> material used for the spill clean-up was not found in the tote(IMG 3046).According to the Sally Beauty Holdings,
<br /> Inc. Hazardous Waste Training Guide, the item spilled and used absorbent and gloves are to be bagged and placed
<br /> in the correct hazardous waste bin, and logged in the hazardous waste binder(IMG 3063, 3070).
<br /> -The following was found in the Oxidizers HW tote(IMG 3049):
<br /> -Spillage of what appeared to be a yellow wax was found at the bottom of the tote, however absorbent material
<br /> used for the spill clean-up was not found in the tote(IMG 3055).According to the Sally Beauty Holdings, Inc.
<br /> Hazardous Waste Training Guide,the item spilled and used absorbent and gloves are to be bagged and placed in
<br /> the correct hazardous waste bin, and logged in the hazardous waste binder(IMG 3063, 3070).
<br /> -The hazardous waste(HW)items listed below were found in HW totes with labels that did not include these items,
<br /> nor items of similar composition, as contents of the totes, nor were the items on the Sally Beauty Holdings Inc.
<br /> "Hazardous Material Management"poster.When asked how a waste determination is made for such items,
<br /> Francisco Valle,the store manager, stated he asks his manager which tote the item should be placed in. Francisco
<br /> was not familiar with the Sally Beauty Hot Line number listed on the"Hazardous Material Management poster to call
<br /> if questions regarding waste determinations came up.Also, Francisco did not know about the Sally Beauty SDS
<br /> Application (IMG 3104),which provides product Safety Data Sheets(SDS)to assist in making waste
<br /> determinations.
<br /> -A bottle of Gigi Slow Grow with Argan Oil hair minimizer was found in Corrosive Acids HW tote(IMG 2969).
<br /> -A bottle of Gigi Pre Hon pre-wax cleanser was found in the Corrosive Acids HW tote(IMG 2970).
<br /> -A bottle of Gigi Post Wax Cooling Gel skin freshener was found in the Corrosive Acids Hw tote(IMG 2971).
<br /> -A bottle of Gigi No Bump Solution skin smoothing topical solution was found in the Corrosive Acids HW tote(IMG
<br /> 2974).
<br /> - Four bottles of Toppik Hair Buidling fibers were found,two in the Corrosive Acids HW tote(IMG 2980-2983), and
<br /> two in the Toxic&State Regulated HW tote(IMG 3027, 3029).
<br /> -A bottle of BTZ Beyond The Zone Rock On Volumizing Powder was found in the Aerosols, Flammables, &
<br /> Combustibles HW tote(IMG 2992-2993).
