"Hazardous Material <br /> Management"poster(The Poster)posted on a wall above the HW store area(IMG 3306): <br /> -A bag labeled as"Leaker 627003 damaged in shipment Cirepil blue beads,"was found in a Toxic&State <br /> Regulated (State Toxic) HW tote(IMG 3316, 3322-3323).According to the store manager, Samantha Mustain,the <br /> bag contained Cirepil blue wax beads by Perron Rigot.The wax beads were wrapped in absorbent inside the bag <br /> (IMG 3316). The Safety Data Sheet(SDS)for this item was in French and could not be read by Samantha(IMG <br /> 3317-3319). <br /> -A container of Gigi All Purpose Hard Wax was found in a State Toxic HW tote(IMG 3322, 3333-3335). <br /> When asked how a waste determination is made for such items, Samantha stated she calls and asks other <br /> Cosmoprof store managers, to determine which bin an item should be placed in. Samantha did not appear familiar <br /> with the Sally Beauty Hot Line number listed on The Poster to call if questions came up.Also, Samantha stated she <br /> had not used the Sally Beauty SDS application on the store's Ipad until today's inspection, nor was she familiar with <br /> how to use the application. <br /> A generator shall make a hazardous waste determination and keep a record of any test results,waste analyses, or <br /> other determinations made in accordance with hazardous waste regulations for at least three years from the date <br /> that the waste was last disposed of. <br /> Immediately locate a copy of the hazardous waste determination made for any waste not already listed on the <br /> "Hazardous Materials Management" poster, ensure store employees are familiar with these waste determinations, <br /> and submit a copy to the EHD.
