Maintenance and monitoring records for the last three years were not found on site. Missing records include a work order for the response to the L5 91 diesel annular sensor fuel alarm on 1/25/18. These records shall be maintained on site for at least three years. Monitoring records include: (1)date and time of all monitoring or sampling; (2)monitoring equipment calibration and maintenance records; (3)results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5)logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6)results of inventory readings and reconciliations. Immediately locate and maintain all missing maintenance and monitoring records for the last three years on site and submit copies to the EHD. This is a minor violation. 202 CCR 2715(c)(3), 2715(c)(4) Failure to maintain the"Facility Employee Training Certificate"on-site(after October 1, 2018). -The Facility Employee Training Certificate for 12/26/18 contained no CERS ID, no zip code, and date of training. -The record of designated operator training conducted 11/15/18 was on an outdated form. All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall be completed and shall contain accurate information for all employees trained. Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. This is a minor violation. 204 CCR 2712(i)Current monitoring plan approved by the EHD not found on site. An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a minor violation.
