The designated operator visual inspections shall be conducted within 30 days or <br /> less of the previous inspection. Ensure that all designated operator inspections are performed within the required <br /> time frames. <br /> The designated operator failed to check and note all required items on the visual inspection reports.The incorrect <br /> and missing information included the following from the inspection reports dated 10/31/18, 11/25/18, 12/26/18, <br /> 1/15/19, 2/19/19, 3/18/19, and 4/8/19: <br /> -No CERS ID listed. <br /> -Section III was left blank. <br /> -Section V was left blank. <br /> -The question regarding UDC sensor locations was left blank. <br /> -An incorrect monitoring system certification date of 5/2/18 was listed. <br /> -An incorrect spill bucket testing date of 5/2/18 was listed. <br /> -An incorrect overfill prevention test date of 9/26/18 was listed. <br /> -An incorrect secondary containment test date of 5/4/18 was listed. <br /> -Tank and line integrity testing is incorrectly marked"No". <br /> -The owner/operator signature is missing. <br /> -The designated operator signature is missing. <br /> -The reports state that containment sumps were opened and looked inside, but left blank the question regarding <br /> containment sump sensor locations. <br /> -The N and NA answers were not documented in Section III. <br /> -The overfill prevention testing was left blank. <br /> During the visual inspection,the designated operator shall check that all required testing and maintenance for the <br /> UST system have been completed and shall check all required items on the report. Ensure that designated <br /> operators performing visual inspections at this facility are including all of the required information on the reports. <br /> This is a minor violation. <br /> 313 HSC 25290.1,25290.2, 25291 Failure to construct, operate, and maintain primary containment as product-tight. <br /> Per technician, Felix Ramirez, a leak was found at(east side)dispenser 7/8 and the float and chain was tripped <br /> prior to his arrival, indicating a leak in the primary piping. All primary containment for the UST system must be <br /> tight. Immediately have a properly licensed,trained, and certified contractor repair or replace the failed component <br /> under permit and inspection of the EHD. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by May 12, 2019. <br /> The monitoring system certification was conducted 1/15/19 and was not witnessed in its entirety by EHD.
